|Registration for the upcoming school year 2017-2018 will now be completed online via our district website. There will be no paper forms to complete as it will all be done electronically, however you will still need to provide the necessary documents to the campus registrar to complete registration.
The following documents are required to complete enrollment:
Proof of Residency (Mortgage or Lease Agreement, Electricity, Water or Gas bill)
Proof of Identity of Parent/Guardian (Driver’s License, Passport, State ID, etc.)
Student’s Birth Certificate or any other legal document that establishes identity (Passport, Driver’s License-if applicable, hospital birth record, adoption record, etc.) A certified copy or original birth certificate is needed if the student is under the age of 11.
Student’s Social Security Card
Providing the social security card is voluntary.
Refusing to provide will not prevent your child from enrolling/attending school.
If a student does not have a social security number, SMSD will assign a state-issued number which will only be used for state reporting purposes.
Student’s Immunization Records (Must be current.)
Last Report Card/Withdrawal Form from Previous School

Step 3: Set an Appointment with the Campus Registrar
Once you have completed Step 1 and 2, please call the campus registrar to set up an appointment to finalize registration. For your appointment, please ensure you bring in all the required documents listed above to complete registration.
